Please Note: Effective August 1,2008: Credit Bureaus require landlords or businesses wishing to receive credit reports to have an on-site inspection, have a publicly listed telephone number and a business license. You must meet ALL requirements to qualify for service.

REQUIREMENTS

• Business/Contact Information
• Service agreement signed by principal/signatory, and a copy of their driver's license
• Copy of Articles of Incorporation OR Incorporation Number (must include State incorporated in)
• Copy of your current business lease to include length, starting and end dates
• Copy of Business License AND/OR Copy of the Articles of Incorporation and Articles of Partnership, a copy of the Federal Tax ID Certificate or a copy of the State Tax ID Certificate Note: Application for certificate is not acceptable documentation
• Copy of Broker License (Real Estate)
• Copy of Phone Listing (If too new to be listed, please include a copy of your most recent phone bill)
• Completed Physical Inspection (a fee may be required)
